  • there is a $4,000 calendar year in-network maximum 
  • there is a $3,000 calendar year out-of-network maximum
• Voluntary plan whereby employees must enroll within 35 days from date of hire or during Open Enrollment using Benefitfocus
• PGCPS pays 75% of monthly premium for dental, the PGCPS share increases to 80% after 8 years of service. The employee pays the remainder through payroll deduction
• Premiums are deducted on a pre-tax basis
